Life gets busy. Between work, family, and everything else on your plate, it’s easy to push your own health to the side. But taking a little time each year to check in with your doctor can make a real difference — and Monument wants to recognize that.

In 2026, you can earn up to $200 just by completing your annual checkup. If your spouse or domestic partner is also eligible, together you can earn up to $400. No challenges. No tracking. Just a simple visit focused on you.

A yearly checkup is a chance to pause, ask questions, understand your numbers, and stay ahead of potential issues — even if you’re feeling great.

Getting started is easy:

• Schedule your annual checkup with your primary care provider or a Heritage Employee Wellness Center
• Bring the Wellness Incentive Claim Form (if Applicable)
  • Employees visiting their primary care physician or the Brandenburg, KY, wellness center will need to bring the form on page 2 to their appointment, complete Section 1, and ask their doctor to complete Section 2 and return it to them.
  • Employees visiting THG wellness center in Indianapolis or the Next Level centers in Texas do not need to bring a copy of this form. The wellness center will submit on your behalf.

That’s it. Once your visit is confirmed, the incentive will be added to your paycheck as soon as administratively possible.
